Resuscitation with intraosseous lines in neonatal units.

چکیده
Two infants on the neonatal unit aged 143 days and 47 days with difficult venous access were successfully resuscitated from episodes of coliapse using the intraosseous route. (Arch Dis Child 1993;68:324-325) Intraosseous resuscitation of babies and infants is taught to residents in North America in the paediatric advanced life support course.1 No such formal training is widely available in Great Britain. Residents are encouraged to site intraosseous lines in an arrest if conventional venous access has not been established within one minute. The chosen route is in to the medial tibial plate (figure). The needle is simply inserted using a screwing action at right angles to the bone surface, when the marrow cavity is entered the needle gives. Correctly positioned, the needle is very stable and fluid can be infused without resistance. Case reports Recently two babies on our neonatal unit have collapsed and venous access was impossible. Both had had multiple peripheral cannulations and previously attempted central catheterisation.

Intraosseous lines in preterm and full term neonates.
AIM To evaluate the use of intraosseous lines for rapid vascular access in primary resuscitation of preterm and full term neonates. METHODS Thirty intraosseous lines were placed in 27 newborns, in whom conventional venous access had failed. RESULTS All the neonates survived the resuscitation procedure, with no long term side effects. CONCLUSION Intraosseous infusion is quick, safe, and ef...
متن کاملThe utilisation of intraosseous infusion in the resuscitation of paediatric major trauma patients.
Intraosseous lines are a reliable and rapid tool for obtaining vascular access in emergency situations, particularly in children. Their use is recommended when intravenous access cannot be easily secured and there is a need for fluid or pharmacological resuscitation. Training in this technique is included in the Advanced Trauma Life Support (ATLS) and Advanced Paediatric Life Support course (AP...
متن کاملEfficacy and safety of the EZ-IO™ intraosseous device: Out-of-hospital implementation of a management algorithm for difficult vascular access.
OBJECTIVE Intraosseous access is a rapid and safe alternative when peripheral vascular access is difficult. Our aim was to assess the safety and efficacy of a semi-automatic intraosseous infusion device (EZ-IO) when using a management algorithm for difficult vascular access in an out-of-hospital setting. METHODS This was a one-year prospective, observational study by mobile intensive care uni...
متن کاملEvaluating Neonatal Resuscitation Skills of Nursing and Midwifery Students Using Objective Structured Clinical Examination (OSCE)
Background & aim: Neonatal resuscitation training is mandatory for the staff providing maternity and neonatal services in order to ensure competence during the academic period. This study was conducted to assess the neonatal resuscitation skills of nursing and midwifery students. Methods:In this study, the skills of 48 nursing and midwifery students were assessed, using Objective Structured C...
متن کاملVascular access in resuscitation: is there a role for the intraosseous route?
Intraosseous vascular access is a time-tested procedure which has been incorporated into the 2010 American Heart Association Guidelines for Cardiopulmonary Resuscitation. Intravenous access is often difficult to achieve in shock patients, and central line placement can be time consuming. Intraosseous vascular access, however, can be achieved quickly with minimal disruption of chest compressions...
